- Proof of Student's Identity (example: Child’s Birth Certificate)
- Proof of Parent/Guardian Identity
- Child's Immunization Record
- Complete Preschool Packet (including Health Statement)
- Submit Records Request Form

- Now that you are enrolled in Free Horizon, you are a member of our CSN! The Community Support Network (CSN) is FHM’s version of a PTO. The CSN does so much to support our community, from fundraising to hosting community building events.
